Your Pre-registration Form has been submitted

A copy of your pre-registration form has been sent to the email address that was provided. Feel free to print out your form and bring it with you to your appointment. Please make sure that you have your church membership number and state of Florida-issued ID ready to present upon arrival. All participants must wear masks while on-site.

Check our information page, rolcm.org/registerme, for up-to-date details on this pilot program. A staff member from FEMA or Rivers of Life Church Ministries may contact you should their be any questions or concerns regarding your registration.
