This form must be completely by any participant in any and all activities, (hereafter referred to as “Event”), sponsored by, through, or in connection with Rivers of Life Church Ministries, Inc., and agents, personal representatives, successors and assigns, (hereinafter referred to as “Released Parties.”)

It is mandatory for each minor to submit a fully executed form, signed by a parent or legal guardian (on the signing party and/or the minor’s behalf and on behalf of the signing party and the minor’s heirs, personal representatives, successors, and assigns).

It is the role of the Ministry Head or designated Program Director to ensure that a properly executed form is completed for each participant prior to the commencement of a given activity or “Event”.
